A rhinoplasty, commonly called a nose job, is a surgical procedure that changes the shape and, often, the size of your nose. If your nose has a prominent bump, crooked bridge, or wide tip, or it seems too big (or even too small) in relation to the rest of your facial features, surgical rhinoplasty could be your best option to correct it. It’s sometimes combined (and often confused) with a septoplasty—the surgical correction of a deviated septum, a condition where the wall between your nasal passages is crooked. A septoplasty is performed to improve breathing, while a rhinoplasty is usually performed for cosmetic enhancement.  Both procedures can be performed simultaneously under one anesthetic, with one recovery period.
If the repair or revision of the nose is extensive, then the operating time is longer and there will be more expense involved. The fee is also based on the geographical location. For example, the cost of any facial cosmetic surgery in Hollywood, Beverly Hills, will be more expensive than the same procedure here in Toronto. The other factor and the most important consideration is the experience of the facial plastic surgeon performing the operation. As a facial plastic surgeon gains experience and clientele, their fee is usually more than that of a novice.

After the patient is sedated, the surgeon creates tiny incisions inside the nostrils or on the columella. Then he or she carefully lifts the skin to access the underlying bone and cartilage. The surgeon can then remove or graft tissues, as needed. Typically, a doctor uses conservative methods to minimize the impact to the surrounding tissues while still achieving the desired results. When the reshaping process is complete, the doctor lays the skin back down over the new contours of the nose and closes the incisions.
Case 29: Major concerns for this patient were widening of the tip with a marked lack of projection which made his nose feel flat and washed out. Nostril thickness and rounding was also something he wanted addressed. In this case, irradiated donor rib cartilage was used to project and support the tip along with alar base reduction removing a significant amount of nostril flare to create a natural tip and nostril contour that is ethnically appropriate. Fat transfer to the cheeks helps to balance out his flat mid-face to strengthen the cheek contour which can be seen nicely on the profile view.
6)What else is included with your surgery?  Some are bare bones deals - you get your implants and good bye, while others include a comprehensive pre-surgical assessment, and a thorough post-surgical follow up.  (we've heard from other patients about their friends getting their BA elsewhere, and after the surgery they saw their surgeon only at one week after surgery and never again!  We don't do that ... at our clinic you are welcome to come back and see us as often as needed, and we have a schedule of follow up visits for a full year after your surgery to ensure you get the best results possible.)
Reduction rhinoplasty is one of the most common types of nose reshaping surgery. This procedure can give a patient a nose that is more proportionate to the other facial features. In particular, it can address bumps on the bridge of the nose, an elongated nasal tip, or excessively flared nostrils. The doctor removes small amounts of bone or cartilage to achieve the desired results. To reduce the size of the nostrils, the doctor may need to use a technique called alar base reduction. During this process, he or she removes small wedges of tissue from the area where the nostrils meet the cheeks.
If you don't know what nasal packing is, consider yourself lucky. Nasal packing is gauze placed deep inside the nose, usually to prevent or stop bleeding. Dr Rival uses nasal packing in only about 5% of all his rhinoplasty surgeries. Sometimes in very deviated noses he may place plastic splints inside the nose to maintain a straight septum for 5 to 7 days.

Case 61: The concerns in this case were crookedness and a significant breathing issue due to a severely deviated septum. She also felt her nose was over-projected and a little too big for her face. Here we can see resolution of her crooked septum on base view. The tip has been defined and de-projected and the bump brought down to create a naturally pretty and more balanced contour.
Case 87: This young woman had broken her nose and was noticeably crooked with poor nasal breathing. In addition, she disliked her nasal hump and length. She preferred an aesthetic with a slight supra tip break to soften her profile. Beyond straightening and improving her breathing, you can see how we were able to remove the nasal hump and lift her tip to transform her look while still looking completely natural.
Rhinoplasty is unique in that the final results of the surgery can take up to one full year to develop. Side effects such as swelling and bruising around the nose and eyes can hinder the appearance of the nose for the first two to three weeks. While more obvious changes may be visible in the first weeks following surgery, most individuals will need to be patient in regards to the final outcome. In the months following surgery the nose will begin to settle in to its final shape. Because the final results of rhinoplasty take a significant amount of time, doctors usually discourage patients from undergoing revision rhinoplasty until a full year has passed.
When interviewing doctors, prospective patients should ask them about their approach to rhinoplasty. It is important to choose a doctor who addresses the airways, as well as the external structure of the nose. Even if a patient is undergoing the procedure for purely cosmetic reasons, the doctor should work to protect the nasal passages so that the patient can maintain normal breathing patterns.

Case 38: This beautiful young lady is an early 6 month example of a finesse rhinoplasty. On front view you can see the bridge is narrower and the sense of hang is improved. On profile, the tip looks undone, natural and less projected with correction of the slight hanging columella. The result is a beautiful, natural look that corrects the issues but leaves her looking totally natural and undone.
